This case demonstrates the protocol of Simultaneous implant placement with vestibular bone wall recovering complicated by inflammatory process in area 3.6.

Flapless techniques are most predictable for long term outcome even if we have to work in infected bone wound. In such cases, it is critical to perform non-traumatic molar extraction in chanks and harvesting of infected tissues and thoroughly disinfect the wound.

Step by step protocol:
1.Fragmentation and non-traumatic tooth extraction.
2. Socket management, removing the infected tissue with good washing by solution of metronidazole.
3. Preparation the bone site.
4. Insertion the bone graft and placing wide implant.
6. Using digital protocols to produce temporary crown.
7. After 4 month placing the permanent crown.
